New Client Forms

To ensure a smooth onboarding process for new clients, we require the completion of two forms: a Mindbody form and credit card authorization form. Please note that appointments will not be confirmed until both forms are received. We appreciate your cooperation and look forward to working with you. Sanela Salon gives the assurance to our clients that their personal information will not be shared with any third parties.

Sanela Salon requires a new client to complete and sign authorizing use of your credit card that you have provided to make a reservation or if you fail to cancel within the appropriate time. Doing so says you agree to our pricing and / or cancellation policy. Sanela Salon has a 24 hour cancellation policy. You will receive a reminder phone call, text and / or email 48 hours in advance, however, if you need to cancel your appointment but do not do so prior to 24 hours before your appointment, you will be charged the full price of the services booked.
